participate in organized sports?

Should my patient with First-Degree AV Block participate in organized sports?

Need to know:

Asymptomatic, no structural heart disease

"Asymptomatic athletes with no structural heart disease and first-degree AV block (PR interval < 0.3 ms) can participate in all competitive sports unless there are findings that indicate that the person is at risk for progression to higher-degree block that would symptoms."

All sports = acceptable unless findings indicating risk for progression to higher-degree block

(Class I Recommendation; Level of Evidence C)

Asymptomatic, type 1 second-degree AV block with exercise

"Asymptomatic athletes with first-degree AV block, in whom type I second-degree AV block appears with exercise, should be evaluated further for possible intra-His or infra-His block with EPS."

Should undergo EP study

(Class I Recommendation; Level of Evidence C)

Structural heart disease

"If structural heart disease is present, athletic restrictions should be recommended as appropriate for the type of structural heart disease."

Depends on structural heart disease

(Class I Recommendation; Level of Evidence C)
